What You Need to Know
- Your records are available. Your dental records have been made available to the Gentle Dentist – Greenwood team so we can continue your care.
- Scheduling is easy. If you need to schedule or change an appointment, you may call Dr. Majid’s former office number at (317) 300-6157, which now forwards to the Gentle Dentist Greenwood office, or call us directly at (317) 215-7601.
- Visit our Greenwood office. All future appointments will take place at Gentle Dentist – Greenwood, located at 1642 Olive Branch Parke Ln Ste 700, Greenwood, IN 46143.
- You always have a choice. You are free to receive dental care from the provider of your choice. If you would like your records transferred to another dentist, please call (317) 300-6157, and we will assist with the transfer upon your authorization or as otherwise required by law.
